Personal Injury Claims

How do I make a personal injury claim?

  1. Access a current Personal Accident Claim Form. Claim Forms are available via the following methods:
    • Download – by downloading the relevant claim form: You will need to send the claim form to us with any medical accounts for reimbursement;
    • Phone – by phoning Echelon on 1800 640 009 (this is a toll free number from anywhere in Australia). Claims staff will send a claim form to you for completion.
  2. Complete all relevant sections of the Claim Form.
    • Your claim form may be returned if there is important information missing.
    • For assistance contact Echelon on 1800 640 009.
  3. Send your completed form to Echelon as soon as possible within 180 days from the date of injury.
  4. Echelon will confirm receipt of your claim or make contact with you should they require more information.
  5. Echelon will confirm receipt of your claim and/or make contact with you should they require further information.

Important Information

The Health Insurance Act (Cth) 1973 does not permit the Trustee or Insurer to contribute to any charges covered by Medicare (including the Medicare Gap).

* Please note – MRI scans are generally claimable through Medicare, however please check with your referrer and/or provider to confirm if this is the case prior to lodging your claim.

In addition, there will be no refund in respect of:

  1. any expenses recoverable by You from any other insurance scheme or plan providing medical or similar coverage or from any other source except for the excess of the amount recoverable from such other policies/plans.
  2. any expense to which the National Health Act (Cth) 1953 or any of the regulations made there under apply.

Hypothetical example of how to make a Non-Medicare Medical Claim

  1. As soon as possible following the injury, the player should obtain a JLT Sport Claim Form via the this website or by contacting 1800 640 009.

    Section A of the form must be completed by the player and Section B must be completed by the club. The completed claim form must be submitted to Echelon within 180 days of the date of injury.

  2. Have the surgeon or treating physician complete and sign the Physicians Report and Incapacity to Work Statement within the claim form. When all sections are completed, post or fax the claim form to Echelon.
  3. Should the player have private health insurance, it is a requirement to claim expenses covered by the private health fund first. A Statement of Payment should be obtained from the private health insurer and forwarded to Echelon.
  4. Following treatment, post all non-Medicare accounts to Echelon, or alternatively post them periodically throughout the duration of treatment. Do not send in any accounts that are partially or wholly claimable through Medicare.
