Upgrading Cover

What does upgrading cover mean?

From January 1st 2008, all clubs receive the basic level of cover for Non-Medicare Medical benefits. This basic level of cover is called “Bronze”. Clubs can choose to upgrade from Bronze to a higher level of cover to provide their players with increased benefits and reimbursements of Non-Medicare Medical Expenses.

Clubs may also choose to purchase Loss of Income cover for a team or for individuals or both. To purchase Loss of Income Cover, simply follow the steps below and complete Section B of the Optional Upgrade Form.

Upgrading cover is OPTIONAL. Clubs should consider the costs to players and inform members of all decisions made by the club in regard to Upgrading Cover.

AFL Masters Club do not automatically have Personal Accident Coverage. Go to relevant form noted in Step 1 below if you require Non-Medicare Medical coverage for an AFL Masters Club.

How to Upgrade Cover for Teams and Individuals:

1. Step 1 Download and print the appropriate form(s).
   • Optional Upgrade Form for all clubs (excluding AFL Masters / Veterans).
   • Optional AFL Masters Application Form for all AFL Masters / Veterans
2. Step 2 Complete all relevant sections of the form and calculate the total premium.
3. • Section A = Non-Medicare Medical and Capital Benefits Upgrade (Silver, Gold, Platinum).
   • Section B = Loss of Income Cover Purchase (Team or Individuals or both).
4. Step 3 Sign the form, attach payment and send to JLT Sport.
5. Step 4 Upgraded cover will commence from the date JLT Sport receives the completed form and payment.

Please note: JLT Sport is currently developing an Online Upgrade Application system. Registered clubs and leagues will soon be able to Upgrade Cover via this web site. Please refer back to this site later for further developments.

1. Step 1 Download and Print the Optional Upgrade Form.
2. Step 2 Complete all relevant sections of the form and calculate the total premium.
3. Step 3 Sign the form, attach payment and send to JLT Sport.
4. Step 4 Upgraded cover will commence from the date JLT Sport receives the completed form and payment.
